Send us your chargemaster (CDM), CPT / HCPCS code lists and crosswalk pieces, payer fee schedules, a payer contract (new or renewed), policy bulletins, and cost reports (if a rate depends on your cost-to-charge ratio). Set an effective date — the date of service the data applies from — so an estimate for an older claim uses the version that was in force then. One file per item; add a note so we know what changed. Excel, CSV, or PDF. For a very large chargemaster, contact us to arrange a transfer.
